The Ambulatory Care Coordination team provides comprehensive care coordination services to pediatric patients and their families across various specialty clinics and outpatient departments. The team is committed to delivering high-quality, patient-centered care that promotes positive health outcomes and enhances the patient experience. Job Responsibilities:

• Assess and evaluate the needs of pediatric patients and their families to develop individualized care plans
• Coordinate and facilitate appointments, referrals, and follow-up care for patients within the ambulatory setting
• Collaborate with healthcare providers to ensure seamless transitions of care and continuity of services
• Provide education and support to patients and families to promote self-management and adherence to treatment plans
• Utilize evidence-based practice guidelines and standards to optimize patient outcomes and resource utilization
