Job Description - ECMO Specialist-RT (PRN) (2400036X)ECMO Specialist-RT (PRN) - (2400036X)DescriptionThe ECMO Specialist - RT will function as an identified resource to staff in a service area. Provide comprehensive patient care to a variety of patients. Incorporate all members of the health care team in the planning and delivery of patient care. Deal successfully with the most complex clinical problems as they relate to ECMO. Demonstrate expanded knowledge base through participation in program projects and activities. This staff member may be transferred into regular staffing as required.QualificationsMinimum EducationBachelor's Degree in Respiratory or a related field; 4-year program (Required). Or Associate's Degree with at least 5 years of ICU experience can be considered for PRN positions.Minimum Work Experience3 years related clinical experience in a comparable setting (This may be substituted with prior ECMO training for PRN positions only). (Required)2 years ICU experience (Required)2 years Pediatric/neonatal experience (Preferred)Required Skills/KnowledgeAll Respiratory Care Practitioners must demonstrate evidence of application of License with the District of Columbia Regulatory Agency prior to reporting for the first shift of clinical work through a letter of supervised practice. Successful completion of hospital orientation at CNMC. Ability to organize, establish priorities, and communicate effectively, both orally and in writing. Must be decisive and able to perform under tense, emotional, and stressful situations. Must have the ability to problem solve clinical situations including patient conditions and needs. Able to bend/kneel and climb ladders/stairs. Able to coordinate eyes and hands quickly and accurately and clarity of vision at 20 feet or more. Must demonstrate compassion and a customer service orientation when dealing with families, patients, and co-workers. Must have the ability to perform mathematical calculations and have basic computer skills. Other computer skills to include entry level Excel and database management preferred. Must maintain clinical skills and required continuing education credits for license. Maintain Respiratory and ECMO clinical skills and required continuing education credits for department and licensure including but not limited to: BLS, FIT Testing, Respiratory, ECMO, and other mechanical circulatory devices such as CRRT skills review, in-services, etc. as needed. Maintain Respiratory Therapy ICU care proficiencies as needed. Have the physical ability to perform all ICU treatment modalities for Respiratory therapy, ECMO, and mechanical circulatory devices as needed. Have physical ability to move and lift up to 50lbs; frequently endure long periods of standing and walking.Required Licenses and CertificationsRegistered Respiratory Therapist (Required)Basic Life Support for Healthcare Provider (BLS) (Required).Functional AccountabilitiesAssessment and Planning

  • Gather physical and psychosocial data on patient/family and anticipate problems with changes in patient status.
  • Review patient findings with ECMO physician.
  • Contribute to the development of individualized plan of care with patient and family.
  • Review daily parameters with the ECMO physician.
  • Notify ECMO physician of abnormal laboratory values and/or change in clinical status of the patient.
  • Able to access ECMO circuit function and troubleshoot as necessary.Implement Plan of Care
    • Proficient in ECMO, ventricular assist devices, and other mechanical extracorporeal devices skills and operation of equipment as needed.
    • Role model proficiency and completeness of documentation.
    • Role model organizational and prioritization skills necessary to complete assignments within acceptable time frames.
    • Assist in ECMO cannulation and decannulation.
    • Provide rapid implementation of pump priming and initiation of ECMO circuitry in accordance with established standard of care as needed.
    • Assist in integration of other extracorporeal support systems as needed (ventricular assist devices, CRRT, etc.).
    • Assist in implementation or implement, prime, and manage other mechanical extracorporeal devices such as CRRT as needed.
    • Maintain ECMO circuit, monitor patient's clinical status including labs, blood gases and metabolic status while maintaining clinical parameters.
    • Administer blood products using ICU provider parameters with daily parameter settings.
    • Facilitate ECMO equipment review and inventory process as defined by ECMO Leadership team.
    • Support clinical research activities by following established protocols, collecting data, and assisting auxiliary specialists to do the same.
    • Provide regular in-services and updates to auxiliary specialists and critical care staff.
    • Assist with data collection for ELSO, and VAD database, or other data collecting projects.
    • Provide inter-and intra- ECMO transport as needed.
    • Required to work in an assigned ICU as a Respiratory Therapist. In emergent or urgent situations, the Respiratory Therapist may be required to work in other ICU's. The area and modalities are determined by the ECMO leadership team as needed.Plan of Care Program Improvement
      • Work with the ECMO Leadership Team to identify opportunities for daily improvements in patient care.
      • Work on behalf of the team with ECMO physicians providing input into the development of daily plan of care and assists part-time specialists to do the same.
      • Accountable to establish, implement, communicate and evaluate the interdisciplinary plan of care with outcomes.
      • Participate in annual review of ECMO policy and procedure manual as needed.
      • Participate and maintain a project or projects pertaining to the interworkings of the ECMO and the VAD program or quality improvement of patient care approved by ECMO leadership as needed.
      • Assist ECMO leadership team with planning, teaching and implementing clinical and didactic training of new specialists to the team as needed.
      • Assist ECMO Leadership Team with planning, teaching and implementing regular competency review per ECMO policy.Leadership
        • Role model proficiency in ECMO skills, completeness of documentation, and organizational and prioritization skills necessary to complete assignments within acceptable time frames.
        • Support department management decisions and promote a positive work environment.
