Job Title: Transition of Care Nurse
Location: Remote (must live in Northern Virginia)
Schedule: Monday to Friday, 8 AM - 5 PM (Eastern Time)
Duration: 3 months (may be extended)
What you'll do:
Help patients move from the hospital to home or a skilled nursing
facility (SNF).
Requirements:
Must be a licensed RN or LPN.
Must have experience in discharge planning and transitions of
care.
