Reports to: Executive Director
Location: 1118 Light St, Suite D, Baltimore MD, 21230. Hybrid
Schedule: 15 to 30 clients
Status: Part-Time, Hourly
The Position. Rehabilitation Coordinator (Direct Care Staff)
Job Description:
- Responsible for implementing the rehabilitation activities
outlined in the client's Individualized Rehabilitation Plan.
- Responsible for conducting and documenting four (3) hourly
sessions per month for children/adolescents and (1) monthly summary
with a therapist
- Responsible for participation in supervision and training
sessions.
- Responsible for collaboration between client, therapist, and
parent/guardian. Expected Duties:
- Initial Contact with Client
- Contact should be made within 48 hours of case assignment
- Document all unsuccessful attempts on a contact note.
- Initial progress note is due within 24 hours of contact.
- Review and Use of Individual Rehabilitation Plan (every 90
days)
- By the first 3 visits, review IRP with client and parents.
- Understand client diagnosis.
- Understand client goals.
- Identify interventions to assist with the completion of client
goals.
- Collaborate with rehabilitation specialists, therapists,
clients and parents 30 days before the authorization expires.
- Completion of Contact Notes
- Complete session notes within 24 hours of the session being
completed.
- Document all unsuccessful attempts on a contact note.
- Completion of Monthly Summary
- All monthly summaries are due by the 5th of every month for the
prior month.
- Paperwork Compliance
- Supervision and Training
- Meet monthly with team supervision.
- Attend any training and supervision sessions scheduled by the
Program Director.
- Based on the current needs for completion of paperwork the
following schedule will be used to assist with the tracking of
required documents. In the case of non-compliance, an in office
schedule will be implemented.

- Ability to administer various mental health screenings Our
Requirements.
- A bachelor's degree in a health-related field; An associate's
degree in a health-related field;30 hours, or their equivalent, of
college credit toward a bachelor's degree in a health-related
field; or 1 year of work experience in a supervised mental health
setting.
*Must be willing to occasionally adjust hybrid work schedules to
accommodate in person interviews, training, and company events.
