Description
RBH has an exciting new opportunity for a Lead Case Manager to join
our team of Life Changers! The selected individual will perform
difficult professional and administrative work maintaining client
records, providing quality behavioral support, monitoring the
provision of services provided by Case Managers, and related work
as apparent or assigned. Work is performed under the limited
supervision of the Program Supervisor. Supervision is exercised
over subordinate personnel.
Essential Functions
Supervises and oversees the work of Case Managers; coaches,
counsels, disciplines, and evaluates staff performance; tracks
attendance/personal leave; addresses complaints and resolves
problems of staff.
Provides direct case management to clients; develops, implements,
reviews, revises, and monitors individualized service/treatment
plans, behavioral contracts, and transfer/discharge plans.
Provides client services including supportive counseling,
psycho-educational and group counseling, family counseling,
cognitive behavioral therapy, etc.
Assists with intake/assessments, home visits, admission procedures,
initial paperwork, and case assignments; coordinates and gathers
data to assist with admissions.
Coordinates with community partners and facilitates service
linkages.
Provides crisis intervention and prevention services.
Conducts client and community partner outreach activities,
coordinates program support initiatives.
Provides supervision and consultation during training, team
meetings, quality assurance, caseload assignment, individual and
group sessions.
Facilitates biweekly case reviews; conducts chart audits.
Prepares a variety of reports; prepares and maintains client charts
and other records.
Collaborates with principal, CIS, parents, teachers, behavioral
aides, nurse, security specialist and other professionals.
Coordinates public relation activities; establishes and maintains
professional relationships.
Attends meetings, stuffings and conferences as they relate to
client, staff, and program needs.
Knowledge, Skills and Abilities
General knowledge of community and agency resources and programs of
regional and state sources for the community service population;
general knowledge of interviewing and supportive counseling
techniques; general knowledge of social casework techniques;
general knowledge of human development and behavior; general
knowledge of the theories, principles and techniques of individual,
family and group therapy; ability to solve problems within scope of
responsibility; ability to analyze facts and exercise sound
judgment in arriving at conclusions; ability to communicate complex
ideas effectively, orally and in writing; ability to prepare clear
and concise reports; ability to establish and maintain effective
working relationships with clients, associates, community partners,
medical personnel, law enforcement and the general public.
Position Requirements
Education and Experience
Bachelor's degree with coursework in psychology, social work,
counseling, or related field and considerable experience working
with individuals with behavioral health and/or intellectual
disabilities in a behavioral health setting, or equivalent
combination of education and experience.
Special Requirements
Valid driver's license in the Commonwealth of Virginia.
CPI, CPR and first aid certifications.
QMHP-A, QSAP or QIDP.

Starting Salary Range
$57,293.01
